When conjoined twins arrive at the hospital with a cancer diagnoses, Alex recommends a very risky surgery, resulting in a major ethical debate within the department. After spending time with the twins and observing their unique relationship, Alex also begins to question her relationship with Pierce.

When a young man is only able to speak sentences backwards, Alex sees this as the perfect opportunity to test out some new technology. While Jesse agrees this is the best course of action, Millicent believes otherwise. This tension within their department might stem from past history which could further complicate things between Alex and Pierce.

Alex (Melissa George) and Pierce (Dave Annable) disagree on whether an experimental treatment for those with advanced stage cancer is right for the hospital's patients. True to form, Alex takes matters into her own hands with a risky move to find volunteers. Meanwhile, Max (Joshua Leonard) looks to drastically change the family dynamic with Alex, and Ji-Sung (Maya Erskine) connects with Callahan (Jamie Kennedy).

On the anniversary of her sister's death, Alex (Melissa George) performs state of the art heart surgery on a patient who doubts her ability to leave personal grief outside the OR. Meanwhile, Millicent (Shelley Conn) dates a sexy artist, and Pierce (Dave Annable) wants to be a bigger part of Alex's family.

Alex learns that a woman whom she performed a heart transplant on 10 years ago was in an accident. She is brought to the hospital and Jesse and Millicent who also were part of the original transplant try to help Alex. When someone from Alex's son's school calls and she not available, so Pierce goes out to deal with it. And when he takes her son with him and Pierce tries to bond with him but the boy is unwilling. When Alex's patient dies she decides to find someone whom they can transplant the heart to. And they find one, so Millicent goes there to see if the patient meets the criteria problem is, he can't be moved so Alex decides to bring the heart there. But when she arrives, she learns they don't have the equipment needed to perform the transplant. Alex gets a patient who works for Max who is in transition from male to female. And she discovers she has a condition that needs treatment but she's hesitant to do so. And when Pierce raises the question of him and Alex having children together, she says not likely. And Jesse meets a tattoo artist and asks if she can redo a tattoo he has.
